Elder Services introduces the Community Choices Program - "The choice is yours"

Community Choices maximizes the choices available to elders and their families by providing a greater level of in-home services to elders who are at imminent risk of nursing home placement. Community Choices enables elders to continue to live at home for as long as possible.

The Community Choices Program is designed to serve frail elders who are:

age 60 or older;

eligible for MassHealth;

at imminent risk of Nursing Home placement;

need a greater level of services in order to continue to live at home; and

who choose to live at home rather than go to a Nursing Home.
