Caregiver’s organizer, information at your fingertips

 

Elder Services has printed pocket-size organizers designed to assist caregivers in maintaining important information for the loved one in their care such as a list of medications, emergency contacts, a calendar and other vital information to which caregivers need access. Each organizer has Elder Services’ telephone number, address and email, in addition to contact info for other resources such as the Alzheimer’s Association and the Family Caregiver Alliance. There are pages for notes so that the organizer can be adapted for individual’s needs.

With Medicare Part D prescription drug information and other programs, it’s important to organize important information in an easy-to-use pocket or purse organizer.  Caregivers who have sampled the organizer comment that the design provides an efficient way to track information such as the date of recent hospitalizations and medication changes.

These are organizers specifically designed to contain many of the phone numbers of senior centers and Councils on Aging in each area of Berkshire County: North, Central, and South.
