-- Programs and Services -- Contact InformationFree caregivers' pocket organizers
Elder Services has printed pocket-size organizers tailored to different parts of Berkshire County designed to assist caregivers in maintaining important information for the loved one in their care, such as a list of medications, emergency contacts, a calendar, and other vital information to which caregivers need access. Each organizer has Elder Services’ phone numbers, address, and email, as well contact information for resources such as the Alzheimer’s Association and the Family Caregiver Alliance. There are pages for notes making it easily adapted for individual needs.
With seniors selecting their Medicare Part D prescription drug plans, the timing seems right to organize important information in an easy-to-use pocket or purse organizer. Caregivers who have tried the organizer say that the design provides an efficient way to track information such as the date of recent hospitalizations and medication changes.
The organizers specifically designed for either north, central, or south Berkshire County, each containing helpful area information including phone numbers of senior centers and Councils on Aging.
The Caregiver Organizer is provided free to individuals who request them by calling the Council on Aging in their town or Elder Services’ office at 499-0524 or 1-800-544-5242.
