Medicaid recipients twenty-two years of age and older seeking admission to a nursing home must be medically approved (screening) by the Coordination Of Care Department at Elder Services of Berkshire County.

Although most of the nursing home screenings are submitted from the nursing facilities or hospitals, a pre-admission screening from other community members such as a relative, concerned neighbor, home health agency, etc. can also be requested.

During the screening process, options other than placement in a nursing home may be suggested if appropriate.

If someone is determined inappropriate for nursing home placement, a case manager from Elder Services will develop a community based service plan. There is no charge to the client for this assessment.

Again, the Coordination Of Care department determines medical eligibility only. Financial application must be made to the Springfield Long Term Care Office.
