FY 2008 TITLE III SUBGRANTS OCTOBER 1, 2007-SEPTEMBER 30, 2008 Effective October 1, 2007, eleven grants were awarded to the following programs serving seniors in Berkshire County. The Title III grants are authorized under the Older Americans Act and are targeted to meet the priority needs of persons 60 years of age and older. |

Western Mass Elder - Elder Law Project – Western MA Legal Services – Provide representation, advice, and other direct legal services to elders. |
|
Brien Center Chore Services – Provide chore services in the county – oven and refrigerator cleaning, windows and kitchen cabinets washed, air conditioner installation/removal. Limited yard work. |

Gabriele Elston Ferry GNP – In-Home Assessments – Provide in-home health risk assessments and consultation and education about Alzheimer’s disease. |
|
Berkshire Community Action Council Medical Reassurance Service – Calls Berkshire County elders 2-3 times a daily to remind them to take medications. |
| Sandra Doppel, RN – Skilled Foot Care – Provide in-home foot care to elders in Berkshire County |

United Cerebral Palsy – Caregiver Support: Assistive Technology – Support caregivers by providing assistive devices to the elder that promote independence. (Large button remotes, programmable alarms for medication reminders, phones for hearing impaired, etc.) |

Alzheimer’s Association MA Chapter-Caregiver Education – Caregiver Education Program on Alzheimer’s Disease with focus on care at home. Two – 6 week, flexible programs. |

Home Instead Senior Care-Respite Services - Provide respite services for caregivers, individualized to need. |

Lee Visiting Nurse Association-Caregiver Training - Provide two sets of four stand-alone sessions of Caregiver training. |
|
Lee Visiting Nurse Association-RN In-Home Service - Provide one-time in-home visits for care not otherwise covered. |
| Premier Home Health Care-Respite Services - Provide respite relief services for caregivers of individuals affected by Alzheimer’s disease and related disorders. |
