Clinical Assessment & Eligibility (CAE)

Medicaid recipients twenty-two years of age and older seeking admission to a nursing home must be medically approved (screening) by the Clinical Services Department at Elder Services of Berkshire County.

Although most of the nursing home screenings are submitted from the nursing facilities or hospitals, a pre-admission screening from other community members such as a relative, concerned neighbor, home health agency, etc. can also be requested.

During the screening process, options other than placement in a nursing home may be suggested if appropriate.

If someone is determined inappropriate for nursing home placement, the nurse will work with a case manager from Elder Services to develop a community based service plan. There is no charge to the client for this assessment.

Again, the Clinical Services Department determines medical eligibility only. Financial application must be made to the Springfield Long Term Care Office.
